Longest down and distance I could find was fourth and 74, in a 1970 game between the Raiders and Steelers.

First and 10, 7 yard loss.
Second and 17, 16 yard loss
Third and 33, 15 yard penalty after a 6 yard gain
Third and 42, 10 yard holding penalty at the spot of the foul, -23 yards (ouch!)
Third and 65, intentional grounding, 9 yard penalty and loss of down, which may have been a mercy
Fourth and 74, 55 yard punt, which, while impressive, doesn’t make it to the original line of scrimmage.
The Patriots had a fourth and 63 in 1971 as well.
As for the longest sack, Ryan Tannehill was sacked for a loss of 38 yards in 2015, though it involved a fumble.
