Looks like Pfizer's vaccine vials contain more than 5 doses per vial (with some pharmacists reporting up to 2 additional doses)

And good news, the FDA just approved use of the overflow which means more people will be able to be treated.

Though I wonder what happens for the second dose.  I assume there's no guarantee that the extra doses will be distributed exactly the same.

The overfill is by design.  You want to make sure that if you need 5 mg of a drug, but a small amount gets doesn't get pulled into the syringe, you can still administer a 5 mg dose.  It is a bit surprising that the overfill was enough to get a whole additional dose, though.

If there's an extra 1.6 doses in each of 2 vials, can they be combined for a total of 3 doses?

I suspect OWS didn't deliver enough syringe kits to take advantage of it.

Unless I'm misreading, the FDA and Pfizer do not recommend mixing vials.

Correct, you shouldn't mix vials, as it doubles the risk of error or contamination.  But at a time of vaccine shortage, the risks may be worth the (small) extra risk.

You don't have to use the syringes provided by OWS.
