Has there been an NFL new hire in the last two decades who was coach/GM right off the bat? I know of a few who do it currently, but I don't think any of them had the combined role immediately.

Hmmmhhh.... I think there were a few but I'm not sure how they worked out. Two current ex-college coaches are effectively GM and Head Coach despite non-standard organizational structures.
This is from the Chip Kelly wiki page,

Of course Carroll had two previous stints as an NFL head coach so his situation is different.

I've always been under the impression that Carroll isn't really the GM, but maybe it's more that the other guy handles the money side and Carroll is the chief scout.

There's folks here who know more about Seattle's structure than I do but I believe you have it right. John Schneider is the Seattle GM but was hired by Carroll and defers to him on talent evaluation.
